Pet Intake Forms

All required paperwork must be completed either prior to arrival or at the time of check-in. Clients may choose to visit the facility in advance to collect the necessary forms, complete the paperwork during their first visit, or download and print the forms using the links provided below.

Vaccination Requirements

For the safety and well-being of all pets, we require that all dogs and cats be current on their vaccinations. Vaccination records must be submitted prior to drop-off. Please note that no exceptions will be made.

All Dogs Are Required to Have:

• Rabies
• Distemper
• Parvo
• Negative stool sample including Giardia screening results must be provided
• Bordetella Vaccine (Canine Cough) must be done 10 days or more before boarding if the dog has not had the vaccine before (Vaccine is also required every 6 months

All Cats Are Required to Have:

• Rabies
• FVRCP (Distemper)

All Avians Are Required to Have:

• No Vaccinations are required at this time.

All Reptiles Are Required to Have:

• No Vaccinations are required at this time.

All Exotics Are Required to Have:

• No Vaccinations are required at this time.

Facility Tours

We are pleased to offer complimentary facility tours Monday through Saturday, from 10:00 AM to 3:00 PM. We’re confident that after your visit, you’ll understand why we’re a favorite within the pet community. For the safety and comfort of all, we kindly ask that you leave your pets at home during the tour.

No Call, No Show Reservations

Reservation deposits and cancellation fees are not required. However, a $50 fee will be applied for "No Call, No Show" reservations. To avoid this charge, please notify us in advance if you need to cancel your reservation. Thank you for your understanding.

​Medication Fees​

The administration of up to three medications is provided at no additional cost. Each additional medication will incur a one-time fee of $2 (not charged per day). All medications and supplements must be supplied in their original packaging or containers. To ensure proper administration, manufacturer dosage and instructions will be carefully reviewed and followed.
Please note that we are unable to administer injectable medications, such as insulin or fluids. Thank you for your understanding.

Drop Off & Pick Ups

​Drop-off day is charged a full day regardless of drop-off time.

Pick up your pet prior to 12pm on pick-up day for free. Any pick-ups occurring after 12pm will be charged a full day of boarding. Sunday drop-off and pick-up are assessed a full-day boarding fee.
